Types of Hospice Care

Routine

Our routine private hospice care services are provided to patients where they live. Hospice healthcare professionals can assist patients with pain and symptom management, everyday tasks of living, meal preparation, and other services as needed. Routine care is individualized to each patient and their unique situation.

Continuous

When the patient enters their final days, our continuous hospice care services will be administered, typically lasting 1-5 days. The Interdisciplinary Team (IDT) will consult with you about care options and patient needs. During continuous care, a team member is available 24 hours per day.

In-Patient

When pain and discomfort can’t properly be managed at home, an IPU stay may be necessary. The in-patient care provided is to help control severe pain and stabilize symptoms. IPU stays generally last between 2-7 days with the goal of discharging the patient to their normal living arrangements.

Respite

Respite hospice care services are used when a patient cannot be left unattended for long periods. Our team members are available round-the-clock to provide symptom management and daily living assistance. Respite care is usually limited to no more than 5 days in a Benefit Period. Respite care gives family members a break and is often used for out-of-town occasions like weddings.

FAQs

What is hospice care?

It is a specialized type of care focused on providing comfort, care, and support to individuals who are nearing the end of life. It emphasizes enhancing quality of life and managing symptoms rather than curative treatments. Hospice care services are provided by a multidisciplinary team of healthcare professionals, including nurses, counselors, spiritual advisors, and more. The team works together to address the physical, emotional, and spiritual needs of patients and their loved ones.

Who practices hospice care?

A diverse team of professionals works collaboratively alongside the patient and their families. They address various needs of patients, and can include:

  • Nurses: Registered and licensed practical nurses provide direct patient care, administer medications, assess symptoms, and educate patients and families about the care plan.
  • Doctors: Physicians monitor the patient’s care plan, including managing symptoms, prescribing medications, and coordinating with other members of the hospice team.
  • Social Workers: They provide emotional support and assistance with practical areas such as advanced care planning and accessing community resources. They can also help in understanding programs like Medicare.
  • Spiritual Advisors: They offer spiritual and emotional support to patients and their families, respecting their beliefs and helping them find meaning and peace during the end-of-life journey.
  • Counselors: Mental health professionals can provide therapy to help patients and families cope with the emotional challenges associated with terminal illnesses and end-of-life care.

Who qualifies for hospice care?

When choosing private hospice care, it is important to understand the criteria for eligibility. It involves meeting criteria relating to a terminal illness, as well as having a prognosis of six months or less to live. This has to be evaluated by a physician. Hospice care services are tailored to meet the patient’s individual needs, to ensure that they are comfortable during this stage of life. Patients and their families also need to agree to forgo curative treatments and instead focus on comfort and quality of life.

What does hospice care indicate?

Hospice care services include:

  • Pain Management
  • Symptom Control
  • Emotional Support
  • Spiritual Care
  • Assistance with Daily Activities

Hospice care can be provided in various settings, usually in the patient’s own home to provide maximum comfort. It can also be provided in hospitals and institutions with hospice facilities.

How do you pay for hospice care services?

In almost all cases, hospice care services have zero out-of-pocket costs with insurance. At A & H Hospice, we accept Medicare and MediCal with no out-of-pocket costs. We also accept most private insurance, and you can call us at 818-751-0700 to verify if we accept your insurance. Additionally, veterans who qualify for VA benefits are also eligible for hospice care.

Coming off from hospice care?

Coming off from private hospice care is a decision that can be made by the patient or their loved one if their condition improves or if they wish to pursue curative treatments again. When a patient expresses a desire to revoke hospice care, our team will reevaluate the patient’s condition to determine if they still meet the eligibility criteria for hospice care.
