When diagnosed with a life-limiting illness and a cure is no longer possible, hospice may be a good option to support the patient’s care plan. Our hospice care team provides services in the patient’s residence, whether they live at home or in an assisted living facility. Our hospice care team offers rapid response rates, aiming to provide service within 24 hours once contacted. Hospice qualifications for MediCare and MediCal patients offer zero out-of-pocket costs with A & H Hospice. We also accept many private insurances.

Determining the Need for Hospice Care

For a patient with a terminal diagnosis, hospice may be a good option with any of the following conditions:

  • Health continues to decline even with care

  • The patient isn’t eating or drinking well enough to maintain their weight

  • Weight loss of 10 percent or more in six months

  • Recurring infections and frequent hospitalization 

  • Plus some other less common reasons

Because an exact diagnosis for end-of-life can be difficult to determine, MediCare explains hospice qualifications can be extended to patients for periods of time beyond six months, provided they are re-certified for hospice every six months.
