How Often Is Care Provided?

Routine hospice care at home is not a round-the-clock service, however, patients do have 24/7 on-call nursing support. A & H Hospice in Southern California has rapid response rates and aims to provide needed support within 24 hours of receiving a call. In routine hospice care situations, family members are typically the primary caregiver. The hospice nursing team will create a custom care plan that works with the family members to provide medical support and living assistance when needed. The care plan adapts to the patient's needs as they move through the end stages of life.

What Happens if More Care Is Needed?

When symptoms become unmanageable during hospice care at home, medical emergencies occur, or family members need a break, other levels of hospice care may be needed.

In-Patient Care: To control extreme symptoms of pain and discomfort, patients may stay at an in-patient facility until symptoms are stabilized. The goal of in-patient care is to return the patient home. In-patient stays typically last 2-7 days.

Continuous Care: In times of medical crisis, continuous care may be the best option for the patient and family members. A hospice healthcare professional will be present for at least 8 hours a day. Continuous care is usually limited to 1-5 days.

Respite Care: When family members become overwhelmed or need to attend out-of-town events, respite care offers round-the-clock service. Patients receive up to 5 days of respite care in a benefit period.
